ANALISIS FAILURE MODE AND EFFECT ANALYSIS DAN FUZZY FAILURE MODE AND EFFECT ANALYSIS PADA MESIN PENCETAK ROTI DI PT. ELODA MITRA (DIVISI BAKERY)

Abstract

PT. Eloda Mitra is a company that runs in the field of processed meat and bakery. Failure to the machine is inevitable including PT. Eloda Mitra. Bread dough molding machine experienced 26 incidents of failure in period January – August 2017. Bread dough molding machine is the main machine in the process of bread production and there is only one unit in the factory. Failure that often happen caused losses for company. FMEA or Failure Mode and Effect Analysis is a technique used to define, identify and eliminate failures. Application of fuzzy logic in the FMEA would produce more accurate results. FMEA method assess the severity, occurrence and detectability to determine the Risk Priority Number (RPN) to determine the criticality of bread dough molding machine failures. Fuzzy logic used to determine Fuzzy Risk Priority Number (FRPN), Which is based on the result of previous FMEA. Two Failure that have the highest value of the RPN and FRPN, which are “manual valve is dirty” and “Green roller shifted”, should be a concern of the company management of maintenance and repair so that the severity, occurrence and detectability of this two failure modes can be minimized.
