Abstract
CV Dwi Jaya Abadi is one of leather home industry that produced bags and suitcases with make to order and make to stock systems. In its production process, CV Dwi Jaya Abadi often face the various risks in the form damage or disruption during the production process. Therefore, as an effort to improve the product quality, needed supervisor and controlling during production process to minimize probability of quality risk occurrence. Failure Mode and Effect Analysis (FMEA) is a method that used in this research to identify the risk of failure in the production process. Identification and risk assessment process obtained from interview and questionnaires to the six (6) employees of the company who understand the whole production process. Risk assessment in FMEA is measured by three categories like severity, occurrence, and detection. Risk identification and risk assessment of FMEA results indicated that there are 14 quality risks in bags and suitcases production process. Through probability-impact matrix, can be seen that there are some critical quality risk and must be mitigated. Critical risks include, the risk of complicated and unclear pattern, cutting of material does not fit with the size or specification, the result of skiving materials too thin, dirty gluing and less powerfull, less dense of sewing materials and easy to open, accesories are not installed properly and easily separated. Suggestions of risk mitigation were solved using Poka Yoke method.
